Common Questions about Project PAVE

Who is eligible for Project PAVE?

Project PAVE serves students ages 3 to 21 who reside in Tennessee and who have been identified as visually impaired according to state eligibility guidelines for special education. We also consider candidates based on the likelihood of benefiting from low vision devices. Students are eligible for a reevaluation after three years unless there has been a change in visual acuity or visual field that warrants a reevaluation sooner.

Because Project PAVE does not provide medical care, participants must have had a complete eye examination within 12 months of referral.

I am the parent or guardian of a child with low vision. How do I enroll my child in Project PAVE?

Complete the contact form at the end of this page and return it to your child’s vision teacher (if applicable) or directly to the Project PAVE office.

Please include records from your child’s last eye examination if your child was seen by an eye doctor outside of VUMC.

I am a teacher of a student with low vision. How do I refer my student to Project PAVE?

Have the child’s parent or guardian complete the contact form below. With their permission, please also send copies of any educational records (FVA, LMA, O&M assessments) to the Project PAVE office. The parent or guardian must also send us records from the child’s last eye examination if the child was seen by an eye doctor outside of Monroe Carell Jr. Children's Hospital  at Vanderbilt.

I am a doctor and would like to refer my patient with low vision to Project PAVE. How do I do so?

Have the child’s parent or guardian complete the contract form below and return it to the Project PAVE office. You can also call the office directly at (615) 936-3718. We will need a copy of the most recent clinic note for that child.
