More Information about NICU Bridge to Home
The NICU Bridge to Home program allows babies and families to go home sooner than with a traditional NICU stay. At home your baby will receive ongoing care and support, remotely, from the providers at Monroe Carell. For many families this means bringing their baby home sooner from the NICU, while still getting day-to-day support and check-ins from the medical team.
Bridge to Home continues the education you receive in the hospital, monitors baby’s growth and development and works with baby’s primary care provider for future care.
How the Program Works
- Remote monitoring. Families receive a tablet and Bluetooth scale to monitor their child's health and may receive other equipment based on their needs.
- Daily updates. Use the app to upload daily information about their child’s health, which is monitored by our team.
- Telehealth appointments. We conduct regular telehealth appointments through the My Health at Vanderbilt app to address any worries and provide ongoing support.
- Coordinated care. Your baby’s medical home is with your pediatrician. Our team works with your pediatrician after your baby leaves the NICU. We do not replace the pediatrician.
Who Qualifies for the Program
Bridge to Home is for infants who no longer require NICU equipment but need some ongoing medical help and supervision. The care team evaluates your baby’s readiness to go home. Generally this means when your baby is able to:
- Breathe on their own
- Maintain their body temperature
- Achieve feeding by mouth
One of our team members will discuss Bridge to Home with you and can enroll your child in the program. If you enroll, we will make sure you go home with all the necessary tools to take care of your baby.
