Before your appointment

Please have your physician fax us a signed referral form. Our fax number is: (615) 936-3956.

Care givers should call insurance to determine if medical nutrition therapy is covered.

Bring with you to your appointment:

  • Insurance card
  • Co-payment or payment in full if your insurance doesn't cover the visit

Your visits

It usually requires three visits to get the best results from your nutrition therapy. To reach your or your child's nutrition goals, more than one visit may be needed for education reinforcement and goal monitoring.

Wear loose fitting clothing and slip on shoes so measurements can be taken easily. At each visit there will be a weigh-in, plus other measurements that may be needed. The information given at your visit will depend on your needs, lab work and diagnoses. This may include but is not limited to:

  • Diet analysis
  • Meal plans
  • Enteral feeding management
  • Counseling on food and medicine interactions
  • Special diet instructions
  • Weight gain or loss
  • Tips for eating out
  • Goal setting

Before you leave ask your dietitian for a business card that tells you how to contact them.

After your visit

A follow-up visit will usually be scheduled. Until your next appointment feel free to ask additional questions by calling the dietitian you visited.
